Contact Social Security Administration office a minimum of 90 days prior to your 65th birthday to initiate Medicare process.
You will receive your Medicare card in the mail once application process is complete. We will need to verify account info to the card.
Ensure you have two forms of ID and your Medicare card present for service.
A final ID will not be “indefinite” until after your 65th birthday for dependents.
Sponsor must be present unless a DD Form DD 1172 was completed, the dependent has a general power of attorney, or if the spouse is deceased please bring in the death certificate for processing
